Must have/meet: * Resident of Boyle, Mercer, Lincoln, Garrard or Casey counties * Income guidelines

Required documents

Please bring with you: * Picture ID for each child in household (no copies) * Birth certificates or immunization records of all children in the household under 18 years of age * Proof of household income * Proof of all monthly expenses including rent and utilities (copies of bills, cancelled checks, or payment receipts with name of applicant and address in service area) * Size of children's clothing and shoes and choice for toys for ages 0-17
