Comprehensive Outpatient Substance Use Disorder Treatment

Eligibility
Must have/meet:
  • Male, 18 years of age and older
  • Homeless veteran with a discharge from the service for reasons other than dishonorable (for Veteran's Grant Per Diem Program)
  • Walk-in clients must not have pending legal issues, probation or be on parole
  • Only court orders from Fayette County will be considered and must contact intake for information and application
Required documents
Please bring with you:
  • Picture ID
  • Proof of Military Service-DD214 for Veterans in Grant Per Diem Program
If veteran currently doesn't have either of these, Hope Center staff will assist in acquiring these document
