Description

Provides specialized care for persons facing a life-limiting illness, their families and caregivers in Fayette and Jessamine counties. Services offered include grief counseling, grief support groups, dementia diagnosis and caregiver support, hospice care, palliative care, pediatric palliative care, personal care, in-home primary care for homebound individuals and respite care for caregivers.
