Eligibility

Must have/meet: * Homeless (living on streets, in cars, staying with friends or other family members or living in substandard housing) * Individuals and households may also qualify if they are spending more than half of their income toward rent and utilities * Must participate in case management program

Required documents

Please bring with you: * Proof of household income * Recent pay stub(s) or a letter from your current employer stating employment status * Most recent income taxes * Food stamp award letter (if applicable) * Proof of family size (birth certificates, Social Security numbers, etc.)

Description

Provides healthcare services for homeless individuals and families in Powell and surrounding counties. Offers services that include physical examinations, limited immunizations, office and laboratory testing, care for acute health problems and treatment for chronic health problems. Provides prescription assistance, mental health and substance abuse services, dental services, outreach and support services and nutrition counseling.
