Must have annual income equal to or less than 150% of the poverty income levels set forth in the poverty guideline chart
Required documents

* Driver's License/Picture ID for everyone 18 years old or older
* Social Security cards for everyone in household
* Verification of entire household income by 3 most recent paystubs, SSI/SSDI, child support or benefit award letter
* LexServ bill
