Must have/meet:
* US Citizen or permanent resident
* Resident of Kentucky with no suspensions in any other states
* 16 years of age or older with a permit for 180 days if younger than 21 years old or 30 days if older than 21 years old

* Pass written and vision test (if expired license or permit)
