Must have/meet:
*Resident of Fayette County for 1 year
*Income at or below 50% AMI
*Not received emergency financial assistance within the past 36 months
*At risk of loss of housing, must be at least 30 days behind
*Utilized all personal resources

*Picture ID
*Social Security cards for household
*Proof of residency for 12 consecutive months
*Proof of household income
*Eviction notice, forcible detainer or statement (ledger) from your mortgage company showing past due housing cost issued in last 30 days
