Offers services that include physical examinations, limited immunizations, office and laboratory testing, care for acute health problems and treatment for chronic health problems. Provides prescription assistance, mental health and substance abuse services, dental services, outreach and support services and nutrition counseling.

Provides support and assistance; throughout the life cycle; starting with head start for children, continuing through skills training programs and culminating with adult day services for the frail and elderly.
