Please bring with you: * Driver's License/Picture ID (for everyone 18 years old or older) * Social Security cards for everyone in household * Verification of entire household income by 3 most recent paystubs, SSI/SSDI, child support or benefit award letter * Shut-off /default notice * Receipt of $56 reconnect service payment, if applicable * LexServ bill
