* Not have applied for Christmas assistance elsewhere

* Proof of household income
* Proof of rent and utilities within last 60 days (copies of bills, cancelled checks, or payment receipts with name of applicant and a Jessamine County address)
* Size of children's clothing and shoes and choice for toys for ages 0-17
