Must have/meet:
  • Be 55 years of age or older
  • Reside in PACE service area
  • Need a nursing-home level of care
  • Be able to safely live in the community with help from PACE

Provides a comprehensive service package that permits persons 55 and older to continue living at home while receiving services rather than being institutionalized. The PACE program becomes the sole source of services for Medicare and Medicaid eligible enrollees. Provides social and medical services primarily in an adult day health center, supplemented by in-home and referral services in accordance with the participant's needs.
