Description
Provides individual assistance in finding and establishing a relationship with medical and behavioral health services. Acts as a liaison between various providers and their patients. Helps navigate the health insurance system. Helps client fine resources such as (housing, food, supplies, and more). Provides support during pregnancy and patients with chronic disease.
