Description
Provides basic health care including physical examinations, immunizations, family planning, nutrition assistance, diagnosis and treatment of common ailments for low-income persons, those who are experiencing homelessness and those uninsured or underinsured.

Please bring with you: * Picture ID * Proof of residency * Last year's income tax return or proof from the Internal Revenue Service that taxes were not filed last year
Eligibility
Must have/meet: * Resident of service area * 13 years of age or older * Income guidelines * Have a job or have been employed with the last 6 months * Working underinsured that meet income guideline * On disability but do not have insurance that meet income guideline * Live in a sheltered environment * Just released from incarceration within the past 6 months * New to the community for past 6 months
