Special Notes/Funding
On designated days, eligible students are taken on a shopping trip to Ashland's local J C Penney store by Friends of the Children and Family Resource and Youth Services Center personnel to shop for school attire. A student may choose 2 tops, 2 bottoms, a package of underwear, a package of socks, other needed undergarments, an outer garment for cold weather, and a pair of name brand shoes.

Please return to Family Resource and Youth Services Center: * Consent forms signed by parent/guardian allowing student to receive clothing assistance and to go on shopping trip for clothes * Completed Clothing Request Form that is provided by the Family Resource and Youth Services Center

Intake Procedure
Referral must be made by contacting the Family Resource Center or Youth Services Center of the studentâs school to request school clothing assistance; referral can be from a teacher, other school staff, social worker, counselor, other professional, or the parent or guardian of the student
