Description
Provides basic health care including physical examinations, immunizations, family planning, nutrition assistance, diagnosis and treatment of common ailments for low-income, persons who are experiencing homelessness or those uninsured or underinsured.

Please bring with you: * Proof of household income * Recent pay stub(s) or a letter from your current employer stating employment status * Most recent income taxes * Food stamp award letter (if applicable) * Proof of family size (birth certificates, Social Security numbers, etc.)
Eligibility
Must have/meet: * Homeless (living on streets, in cars, staying with friends or other family members or living in substandard housing) * Individuals and households may also qualify if they are spending more than half of their income toward rent and utilities * Must participate in case management program
