Provides a full range of supportive services for terminally ill individuals who are in the final stages of their illnesses and for their families. Services may include medical care, pain and symptom management, home nurse visitation, case management, emotional and spiritual support and grief services for the patient and members of the family.

Description
Provides a comprehensive service package that permits persons 55 and older to continue living at home while receiving services rather than being institutionalized. The PACE program becomes the sole source of services for Medicare and Medicaid eligible enrollees. Provides social and medical services primarily in an adult day health center, supplemented by in-home and referral services in accordance with the participant's needs.
