Provides an assessment of a person's social service needs for individuals and families in emergency/crisis situations and then coordinates the delivery of the needed services by either offering direct services which are available at the CAReS office or by making referrals as needed to other agencies.

The 2023 CAReS Giving Tree Program: Registration is scheduled from October 16 to November 3 for those applying to receive gifts and/or food vouchers from the program. To obtain a Neighborhood Card, the following documents are required: * Completed application * Picture ID for everyone 18 and older in the household * Social Security numbers for all household members * Proof of income for everyone in the household * Last 2 most recent bank statements, if applicable * SNAP benefits award letter, if applicable.

Description

Provides a holiday assistance program in which eligible Boyd County adults in need who do not have children may register to receive a holiday food voucher.

Description

Coordinated entry is a process developed to ensure that all people experiencing a housing crisis have fair and equal access and are quickly identified, assessed for, referred, and connected to housing and assistance based on their strengths and needs.

Description

Provides an initial assessment by a case manager for people experiencing a housing crisis in which a Service Prioritization Score is determined for the client so that they may be placed on a housing priority list in a way that ensures fair and equal access to available housing.

Provides a voucher which can only be offered once every three months to eligible residents that helps pay for their prescriptions.
