SEARCH TERMS: Adult Foster Homes Provides adult foster homes for veterans needing help with day-to-day care. Veterans live with a foster family, have their own room, access to the house, meals, and receive medical care in the home through the Home Based Primary Care team. This program is an alternative for the Veteran to living alone or in a nursing home. Eligibility
Veterans must currently: be homeless (or in imminent danger of becoming homeless), have limited income below the established HUD threshold, and have an identified clinical need for case management. Veterans in transitional housing, residential treatment programs, or who are working, or have disabilities may also be eligible. FEES: Veterans will be responsible to pay a portion of the monthly rent directly to the landlord (equal to 30% of their gross monthly income, with a minimum of $50/month) and the housing authority pays the remaining portion of the rent. PAYMENT METHOD: Private pay (self-pay)
